Professional Cares (PC) and Professional Health Care Network (PHCN) constantly monitor the performance of all subcontracted agencies by attempting to contact every patient within 24 hours of start of care date. It is verified that the patient has been seen, PCP appointment has been scheduled and medication regimen has been reviewed. Patient satisfaction is determined and recorded. This information, along with facility re-admissions, are constantly monitored and evaluated to ensure that patient referrals are directed toward the best performing subcontractors.
An ongoing study with Optum, a large medical network in Maricopa County, reveals that PHCN has had 37% less facility re-admissions in comparison to patients not seen by PHCN.
